The Health Assessment or Kaiser Permanente Health Profile are quick, easy and confidential questionnaires that offer you a snapshot of your health and health risks and helps you create a roadmap to healthy living.
Each year, the Trust offers participants the opportunity to take the Health Assessment or Health Profile and earn an incentive. If you and your eligible spouse or covered domestic partner both complete it between November 1 – December 15, 2024, you could save hundreds on your 2025 medical expenses.

The Health Assessment is a confidential questionnaire about health and lifestyle habits. It’s quick and easy to complete and gives you immediate results to help you find ways to improve or maintain your health. It can also help you save money if you complete it during the incentive period.
If you, and your eligible spouse or covered domestic partner 18 years of age and older, both take your annual Health Assessment between November 1 – December 15, 2024, you may receive the following incentives:
Medical Plan A, B, C, and Z – your annual deductible in 2025 will be $200 less for individual coverage and up to $600 less for family coverage than if you do not take it.
Medical Plan JC28XL – your out-of-pocket maximum in 2025 will be $200 less, than if you do not take it.
The Health Assessment is provided by WellRight, an independent health management service provider. Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with the Trust, your union, or your employer without your permission.
Taking your confidential Health Assessment is quick and easy. If you have health-related information like your blood pressure or cholesterol levels, it would be helpful to gather these numbers in advance so you can get a more accurate result when taking your annual Health Assessment.
To complete both Annual Enrollment and your Health Assessment between November 1 and December 15, 2024:
- Register or log in at nwadmin.com
- Select Annual Enrollment and follow the steps to verify your information
- Choose a medical plan if you are eligible for a choice of plans
- After Enrollment, select the link for WellRight – you’ll be taken to the WellRight site and automatically logged in
- Complete your Health Assessment, selecting “save” when done
Please note: Spouses or covered domestic partners will need to log in using their own login. Also, if this is either of your first time registering, it can take up to 48 hours to be able to access the Health Assessment on the WellRight website.
Throughout the year you can log in and update your Assessment at any time, though you’ll only receive the incentive to save on your 2025 medical expenses if you take it during the incentive period.
Based on your answers to the Health Assessment you may be invited to enroll in one, or several, of the wellness programs offered by the Trust. You may be contacted by phone or mail to participate in the voluntary programs that may be relevant to you.

Wellness programs are paid for by the Trust and provided by independent service providers at no additional cost to you. Any information you share on your Health Assessment or with any one of the programs is completely confidential, protected by federal law, and cannot be shared with your union or your employer without your permission.
The Health Profile for Kaiser Permanente participants is a confidential questionnaire about health and lifestyle habits. It’s quick and easy to complete and gives you immediate results to help you find ways to improve or maintain your health.
Every year you have the opportunity to take your Health Profile, which can help track your health progress, but it can also help you save money.
If you, and your eligible spouse or covered domestic partner 18 years of age and older, both take your Health Profile between November 1 – December 15, 2024, your 2025 deductible will be $200 less for individual coverage and up to $600 less for family coverage, than if you do not take it.
You can complete your Health Profile at any time, but you will only earn the incentive by taking it between November 1 – December 15, 2024. Login or create an account at kp.org/wa to complete your Health Profile.

The Health Profile is an online questionnaire and report that takes an in-depth look at your unique health history and lifestyle to help you and your doctor take better control of your health.
In the questionnaire, you’ll be asked about your medical history and your lifestyle routines. If you receive primary care at a Kaiser Permanente Medical Centers location, certain information from your medical record will already be included, such as your most recent blood pressure reading and cholesterol levels.
After you submit the questionnaire, you will receive an online report with your health status, your risk for certain diseases, and suggestions for improving your health. When you update your profile — every year, or more often if you’d like — you’ll see a comparison to what’s changed since your last profile.
If you have not yet registered for Kaiser Permanente Online Member Services, please follow the instructions on the website. You must register for enhanced services to access the Health Profile.
Once registered, log in and click on Complete a Health Profile. Have your ID card handy—you’ll need your Kaiser Permanente member ID to take the Health Profile.
The Health Profile is administered by Kaiser Permanente. Any information you share on your Health Profile is completely confidential, protected by federal law, and cannot be shared with the Trust, your union, or your employer without your permission.
