Your Annual Health Assessment

Get a snapshot of your health and save on future medical expenses.

The Health Assessment is a quick, easy and confidential questionnaire that offers you a snapshot of your health and health risks while qualifying you for lower medical expenses in the coming year.

Check in on your health and wellbeing.

Each year, the Trust offers PPO participants the opportunity to take a Health Assessment and earn a financial incentive. If you and your eligible spouse or covered domestic partner both complete it when it’s offered in the fall, you could save hundreds on next year’s medical expenses.

  • Medical Plans A, B, and Z — your annual deductible could be $200 less for individual coverage and up to $600 less for family coverage.

  • Medical Plan JC28XL — your out-of-pocket maximum could be $200 less.

The Health Assessment is provided by WellRight, an independent health management service provider. Your responses to the questions are completely confidential, protected by federal law, and cannot be shared with the Trust, your union, or your employer without your permission.

Take your PPO Health Assessment or Kaiser Health Profile based on the plan you were on this year.

How to take your Health Assessment

Take your confidential Health Assessment during the incentive period to save on your future medical expenses.

Taking your Health Assessment is quick and easy.

If you have health-related information like your blood pressure or cholesterol levels, it is helpful to gather these numbers in advance so you can get a more accurate result when taking your annual Health Assessment.

To complete your Health Assessment during Open Enrollment in the Fall:

  • Register or log in at nwadmin.com

  • Select Annual Enrollment and follow the steps to verify your information

  • Choose a medical plan if you are eligible for a choice of plans

  • After Enrollment, select the link for WellRight – you’ll be taken to the WellRight site and automatically logged in

  • Complete your Health Assessment, selecting “save” when done

Questions? Call the Trust at (800) 458-3053 or login to www.nwadmin.com.

Throughout the year you can log in to the WellRight site to update your Assessment at any time, though you’ll only receive the incentive to save on your future medical expenses if you take it during the incentive period each Fall.

Spouses or covered domestic partners will need to log in using their own login. Also, if this is either of your first time registering at nwadmin.com, it can take up to 48 hours to be able to access the Health Assessment on the WellRight website.
