The Kaiser Health Profile is a quick, easy and confidential questionnaire that offers you a snapshot of your health and health risks while qualifying you for lower medical expenses in the coming year.

Each year, the Trust offers Kaiser participants the opportunity to take the Health Profile and earn an incentive. If you and your eligible spouse or covered domestic partner both complete it when it’s offered in the fall, your next year’s medical deductible will be $200 less for individual coverage and up to $600 less for family coverage, than if you do not take it.

How to complete your Health Profile

Taking your confidential Health Profile is quick and easy – and can earn you a lower deductible in the coming Plan year.

You can complete your Health Profile at any time, but you will only earn the incentive by taking it when advertised in the fall. To complete your Health Profile, please register or login to Kaiser Permanente Online Member Services.

If you have health-related information like your blood pressure or cholesterol levels, it would be helpful to gather these numbers in advance so you can get more accurate results.

If you have not yet registered for Kaiser Permanente Online Member Services, please follow the instructions on the website. You must register for enhanced services to access the Health Profile. Once registered, log in and click on Complete a Health Profile. Have your ID card handy—you’ll need your Kaiser Permanente member ID to take the Health Profile.

Your privacy matters

The Health Profile is administered by Kaiser Permanente. Any information you share on your Health Profile is completely confidential, protected by federal law, and cannot be shared with the Trust, your union, or your employer without your permission.
